1. COPY of Birth Certificate for the League to keep
2. 2008 Physical Form
(Due before practice on Monday 14, 2008)
3. COPY of Utility Bill for Proof of City of Tamarac Residency or
COPY of 2008 "Out of City Card" for the league to keep
(May be purchased at the Community Center during registration for $50.00
(Please note that registration WILL NOT be completed without ALL of the above) |
